When it comes to moon phase images such as the one posted here. Does it work in the sense that the moon phase element in the design panel works? Meaning when you choose the moon phase element instead of adding the 8 or 9 ( however many) different images you just add this one image? Or does this kind of function entirely differently then the base element does?

This one image does nothing on its own. To make working moon phase indicator out of it, you either need another images where the moons are covered accordingly, or you need a masking image behind which this one could rotate according to moon phase tag.
